In 1916, Community Nurse Home Care started with one woman who traveled to neighbors’ homes on a bicycle to care for them when they needed it. Ninety-six years later, Community Nurse Home Care has grown to a work force of almost 200 –nurses, therapists, home health aides, social workers – plus a robust volunteer force which includes two therapy dogs!

Our size, our “neighborhood” and the roster of services we offer may have grown and evolved but Community Nurse Home Care has stayed true to our original mission: We bring caring home. And we care for our neighbors when you need us. And with the changes to how health care is delivered, home care is becoming more and more important.

Community Nurse Home Care (CNHC) is among the largest providers of home health care and hospice services in Southeastern Massachusetts. Founded in 1916 by a group of concerned area residents and health professionals, Community Nurse Home Care is a non-profit agency dedicated to providing a continuum of quality home care to the patients in our region. Our licensed professional nurses are highly skilled, compassionate caregivers who oversee a team of health care professionals. Together, they create and manage a comprehensive recovery plan so the patient may remain in the comfort of their own home.

In 1993, Community Nurse Home Care was the first in the area to establish a Medicare-certified hospice program to assist local patients and their families in the final stage of terminal illness. Community Nurse Home Care commitment has always been to offer peaceful, dignified, end-of-life care that focuses on best practices in pain and symptom management.

In 2015, Community Nurse & Hospice Care changed its name to Community Nurse Home Care to better reflect the full scope of services the agency provides – Visiting Nurse, Hospice, Private Care and Wellness.

In 2010, after recognizing  demand was growing for home care services not covered by Medicare, Medicaid or other health insurance, Community Nurse Home Care established a private care division. Community Nurse Private Care caters to the various needs of people looking for private home care services to maintain their wellness and independence at home.   Our professional care providers are able to evaluate dynamic situations and if medical needs change, there is the assurance that the Community Nurse team is already familiar with the patient’s care and is able to serve the increased need.
