Bridge Program
Community Nurse & Hospice Care has a specialized program called Bridge when the focus of care turns from finding a cure to providing comfort. Our Bridge program links traditional home care services with our hospice program. Passage across the bridge is driven by the patient's clinical presentation and psychosocial needs. Our Bridge team consists of clinical staff with advanced training in symptom and pain management that may accompany progressive illness. The Bridge team works closely with the physician to resolve questions about when to request hospice services and help establish a plan of care that fits the needs of the patient and their family. |
