Hospice Services
When curative treatment is no longer an option and a patient wishes to remain at home or in a home-like setting, Hospice care can provide comfort at the end of life. Hospice care can help terminally ill patients with a prognosis of six months or less. End of life care through CNHC promotes best practices in pain and symptom management, and a peaceful, dignified death while maintaining an optimal level of functioning. Once obtaining a request for hospice evaluation, our interdisciplinary hospice team coordinates care with our Hospice Medical Director, Dr. Barbara Pearl, and the patient's primary care physician. Our licensed and certified hospice team provides practical physical, psychosocial, and spiritual support to patients and their families living with terminal illness.
