Our Mission
Community Nurse & Hospice Care (CNHC) is a private non-profit visiting nurse agency that provides the highest standard of home health and hospice for the community. Committed staff work diligently with patients to advance recovery, promote wellness, and achieve independence at home.
Community Nurse and Hospice Care is one of the largest providers of home health services in southeastern Massachusetts. Founded in 1916 by a group of concerned area residents and health professionals, CNHC remains a stand-alone, non-profit institution dedicated to rendering comprehensive home care, education and support to residents in southeastern Massachusetts.
In 1993, CNHC went on to establish the first Medicare-certified hospice program in the area to assist patients and their families in the final stages of many types of terminal illness. Then, in 1997, CNHC opened an office dedicated to serving homebound residents with psychiatric nursing needs.
A new chapter in the long history of Community Nurse began in 2001, with the adoption of our new name to better reflect our expanded service area and to spotlight our hospice services for terminally ill patients. We are now known as Community Nurse & Hospice Care, but our mission remains constant: to provide the communities of southeastern Massachusetts with skilled and compassionate home health care.
Our highly skilled nurses, therapists, social workers, home health aides and volunteers conducted more than 120,000 home care visits last year throughout southeastern Massachusetts. Today, CNHC is recognized as a leader among area home care agencies as an innovator and developer that organizes and spearheads programs to address the needs of our aging population along with the rapidly changing medical needs of your community.
When do I call for Home Health Services?
CNHC can provide home health care to you when you need it, where you need it. Call us to inquire about help with:
- Senior care for an elderly parent/relative
- Assistance making dietary and medication changes after a new diagnosis
- Help with changing surgical dressings and caring for wounds
- Assistance in coping with chronic illness
- Expert physical therapy services to get you back to independent living at home
- Assistance for your loved one with Alzheimer's disease or dementia
- Assistance coping with periods of depression and anxiety
Who pays?
Most insurance companies have a home care benefit as part of their plan. We accept Medicare, Medicaid, and most private insurance plans. As a community nonprofit agency, our goal is to provide care for everyone regardless of age, race, diagnosis or the abitility to pay. To the extent that charitable funds are available, no patient who is eligible will be turned away.
CNHC: A proud United Way funding recipient
Community Nurse & Hospice Care is a proud recipient of funding from the United Way of Greater New Bedford. Funds received from the United Way help to offset the costs of caring for terminally ill patients in their homes.
