Telemedicine
In the United States, approximately five million people suffer from the negative effects of heart failure. According to the American Heart Association, 550,000 new cases are diagnosed each year. In an effort to minimize the complications, provide early intervention for symptoms, and prevent hospitalizations due to exacerbation of heart failure, Community Nurse and Hospice Care now offers telemonitoring in the home. Home telemonitoring is a tool that provides many benefits to patients receiving home health care, including daily assessment of the patient’s medical status and physician access to their patient’s health record via the Internet. Early identification of disease symptoms allows the clinician to consult the patient’s physician and initiate intervention promptly. In addition, home monitoring enables and supports patient participation in managing their health. Some of the most common conditions that would be appropriate for telemonitoring are:
