Nursing/Nursing Management
Pay: $28 – $31
Requirements:
- Graduate of an approved school for licensed practical nursing with at least three years of experience in direct patient care nursing.
- Currently licensed as a licensed practical nurse pursuant to Massachusetts General Laws Chapter 112.
Responsibilities include but not limited to:
- Provides skilled comprehensive nursing care to all ages, genders, and cultural groups.
- To coordinate optimal care and use of resources for the patient/family by communication with the primary nurse, other staff members and other agencies.
- Maintain up to date and accurate records so that problems and changes are reflected as they occur.
- Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support. Accept telephone orders from physicians.
- Assist in patient care by teaching primary caregivers, or by providing direct care as appropriate.
- Meet regularly with nursing staff to review problems or unique issues, share professional support, and exchange feedback aimed toward enhancing professional growth.
- Attend meetings and facilitate discussion of issues from patient care for staff discussion, consultation and evaluation.
- Inform the primary nurse of unusual or potentially problematic patient/family issues.
- Function as associate nurse when responding to patients/families of the primary nurse.
- Assists in providing intermittent care to patients as the need arises.
- Participate in Community Nurse’s orientation and in-service training programs for professional growth.
- Participate in community programs as requested to promote professional growth.
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
- Contributes to the process of Agency Performance Improvement.
Pay based on visit type: $50 – $140
Community Nurse is looking for a per-visit Admissions Nurse to join the Home Care Nursing Team. This position has weekend and holiday rotations.
Requirements:
- Licensed RN in Massachusetts.
- Graduate of an approved school for professional nursing.
- A minimum of one year of recent experience in direct patient care nursing.
- Previous home care experience preferred.
- Experience with documentation in a patient Electronic Medical Record is a plus.
Responsibilities include but not limited to:
- Give skilled comprehensive nursing care to all age, gender, and cultural groups.
- Assess client status upon initial home visit to identify needs in support of development of comprehensive case management plan.
- Document in an accurate and timely manner not later than end of working day.
- Using basic teaching principles, counsel and instruct the patient, family and/or significant other in meeting nursing and related needs.
- Maintain regular communication with the Clinical Manager, and/or other team members concerning patient/family needs.
- Participate in Community Nurse’s orientation and in-service training programs for professional staff.
- Participate in agency and community programs as requested to promote professional growth and understanding of home care.
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
- Accept other assignments as appropriate.
Community Nurse Home Care is looking for a full-time RN Case Manager to join the Home Care Nursing Team. 35 hours per week with weekend and holiday rotation.

Our licensed professional clinicians are highly skilled, compassionate caregivers who create and manage comprehensive recovery plans so that patients may remain in the comfort of their own home.

Scheduling Coordinator is responsible for coordinating and maintaining the daily schedules of clinicians and home health aides, ensuring timely and appropriate coverage for all patients. This position plays a critical role in maintaining continuity of care by matching patient needs with clinician and aide availability, skills, and geographic location.
Requirements:
- High school graduate: preferred candidates will have an associate degree or advanced training.
- Minimum of 1 year of scheduling and administrative staff support.
- Knowledge of basic office procedures and computer data entry.
- Computer experience – Microsoft 365, electronic medical record (EMR).
- Excellent communication skills.
- Requires ability to work independently as well as in a team environment.
