Remote Patient Monitoring (RPM)
Community Nurse Home Care will partner with you and your care providers to monitor your chronic health condition.
- RPM allows real time monitoring of your vital signs(Blood Pressure, Weight, Heart Rate and Pulse Oximetry) between visits with your clinical team
- Customized daily symptom surveys assist with symptom management and timely Intervention
- Video calls allow a nurse to visually assess you and your symptoms
- Customized teaching for you and your caregiver on how to manage your care at home
- Allows you quick access to your VNA nurse through text messaging, telephone and video calls
Care Transitions
Our Care Transitions Program is an extension of our Home Care Services. After discharge from our Home Care, we have the ability to monitor your progress and assist as needed. We touch base bi weekly/monthly to be sure you to continue your journey at an optimum level of health and independence while remaining at home avoiding rehospitalizations.

Hospice Care

Why Hospice
Hospice care is offered to individuals who are terminally ill and are no longer seeking curative treatments.
Life limiting illnesses including: Cancer, Alzheimer’s/Dementia, Chronic Obstructive Pulmonary Disease(COPD), Congestive Heart Failure(CHF), End Stage Heart Disease, Renal Disease, Liver Disease and End Stage Neurological Diseases may benefit from Hospice.
Hospice may be considered for the following reasons:

Palliative Consultations
Our Nurse Practitioner provides Palliative Consultations to people living with chronic or terminal illness. The goal of these consultations is to improve the quality of life for both patients and their families by assisting with pain and symptom management at any stage of a life-limiting illness. Our Nurse Practitioner takes the time to provide in-depth education to patients and families on illness progression and treatment options to help make informed decisions. Supportive Services
Public Health:
Public health focuses on disease prevention and limiting the spread of communicable diseases. We support two area towns with blood pressure clinics and communicable disease surveillance, as well as promoting and protecting the health of those communities.
Memory Care:
Caring for a loved one with dementia can be taxing and overwhelming. Our Certified Dementia Practitioner (CDP) provides resources and support to families in dealing with recognizing the disease, its progression, and next steps, in addition to providing information for community support groups.
Wellness Clinics:
We offer blood pressure screening and health counseling at area elderly housing complexes.
