Impact

When a clinician recognizes that a patient and their family need assistance and education about disease progression and chronic illnesses, they often request a visit from a nurse practitioner. This support helps the patient and their family feel empowered and informed as they make important decisions about their care. 

Patients and families are visited by our hospice clinical liaison to provide information and answer questions about hospice care, whether they are ready for it now or in the future.

Patients and their caregivers receive valuable assistance with education, support, and advice dealing with memory loss and dementia from our certified dementia practitioner (CDP).  

Personalized education is provided for patients and families about nutrition and specialized diets to help with comfort and healing provided by our registered dietition. 

Prior to joint replacement surgery, a patient is visited by our physical therapy staff in collaboration with their physician in their home to explain what to expect after surgery and when they return home for a safe recovery. 

Patients who need equipment for their safety in their home are identified by Community Nurse’s team while providing traditional home health care and hospice services. If the patient does not have the means to purchase this equipment, Community Nurse will obtain it to increase their safety and comfort in the home environment. This includes items such as shower bars, gel cushions or bed rails.

Presentations on nutrition by our registered dietitian and strength and fall prevention activities by Community Nurse physical therapy professionals are conducted at various community locations.
